A domiciliary care worker may assist clients with everyday activities and personal care, depending on the specific role and employer. Duties can include helping with personal routines, mobility, meals, household tasks and other forms of support specified in the employment contract.

For someone interested in the UK care sector, domiciliary care can therefore involve both practical care responsibilities and travelling between clients.

The exact duties will depend on the employer and position. Applicants should always request a detailed job description before accepting an offer.

UK 3-Year Domiciliary Care CoS Opportunity

It is specifically targeted at:

• People already inside the UK
• Female applicants
• Applicants with a full UK driving licence
• Applicants who satisfy the employer’s compliance requirements

The opportunity also states that the COS is intended for a three-year period and that issuance follows compliance.

It is important to understand that a Certificate of Sponsorship is not itself a visa. A COS is an electronic record issued by an approved sponsor, and an eligible worker uses the reference number when making their visa application.

Why a Full Driving Licence Is Required

A full driving licence can be particularly relevant to domiciliary care because workers may need to travel between different clients’ homes.

Unlike a care home position where workers may remain at one location, domiciliary care can involve visiting multiple clients.

For this reason, applicants should confirm:

• Whether they are expected to drive their own vehicle
• Whether a vehicle is provided
• Whether mileage is reimbursed
• How travel between clients is scheduled
• The expected geographical coverage
• Working hours and shift arrangements

Understanding COS 6135 and Care Worker Sponsorship

Care workers and home carers are classified under COS 6135.

Current UK government guidance places specific conditions on sponsorship for care workers. For applications involving COS 6135, the worker must already be in the UK and applying for permission to stay. From 22 July 2025, a sponsor can generally sponsor a care worker only where the worker has already legally worked for that sponsor as a care worker or senior care worker for at least three months, or where the worker has qualifying previous Skilled Worker permission in the relevant care occupations.

This is a particularly important point for anyone seeing advertisements for an immediate care-worker “switch.”

If you are already working for an employer and considering sponsorship, your employment history and current immigration permission need to be checked carefully.

GOV.UK also states that someone switching into the Health and Care Worker route as a care worker or senior care worker must have been legally working in their job for their sponsor for at least three months.

Important: Dependants and Care Worker Applications

The rules concerning dependants are another area applicants should understand.

The current rules generally restrict partners and children from becoming dependants of people sponsored in COS 6135 care worker or COS 6136 senior care worker roles.

There are limited exceptions, including certain people who have continuously held relevant Skilled Worker permission since before 11 March 2024, as well as specific situations involving children.

Therefore, applicants should not assume that a new COS 6135 sponsorship automatically allows dependants simply because an advertisement says dependants are allowed.

Your personal immigration history and the circumstances of your dependants matter.

This is one of the most important points to verify before proceeding with any care-worker opportunity.

Employer and CQC Registration

Another important consideration is the sponsoring employer.

For care workers under COS 6135 working in England, GOV.UK states that the employer must be registered with the Care Quality Commission (CQC).

Who Should Consider This Opportunity?

• Are currently inside the UK
• Are interested in domiciliary care
• Have a full UK driving licence
• Meet the employer’s requirements
• Are legally able to work in the relevant role
• Can satisfy the applicable immigration requirements
• Are prepared to work in Yorkshire

However, eligibility should be assessed individually.

People currently in certain immigration categories cannot switch into the Health and Care Worker route from inside the UK. GOV.UK lists, among others, visitors, short-term students, seasonal workers and domestic workers in private households among those who cannot switch from inside the UK.
